The source for Isabelle's maiden name which was CRADDOCK of England [U.K.] is on p. 160 of the record which indicates her immigration.

~CORRECTION~ Mrs. Isabella HOLDRIDGE's maiden name was not UPHAM which I posted earlier on the genforum genealogy message board. Sorry, for my error.

Mrs. Isabel, Isabella, Isabelle CRADDOCK HOLDRIDGE was the principal witness in a Salem, Massachusetts, witchcraft case against John GODFREY in 1659.

Here is part of my paternal line:
1st generation [earliest]
William HOLDRED b. 1584 England m. _?_ unknown,
2nd generation
William HOLDRIDGE b. London, England, immigrated on the ship, Elizabeth, in 1635 to Ipswich, MA [occupation was tanner] m. before 1640 Isabelle CRADDOCK,
3rd generation
Mehitable HOLDREDGE b. 14 April 1652 Haverhill, Essex, MA, m. 1668 Jonathan SMITH,
4th generation
etc.
